Who is eligible

• For vaccines: Children through age 18 years of age who meet atleast one of the following criteria are eligible to receive vaccines: enrolled in Medicaid, NO insurance, American Indian/Alaskan Native, children with insurance that does not cover vaccines, or private insurance supplemented with Medicaid..
• For records: any age

What to bring

Please bring the following:

• Birth certificate or passport
• Immunization record (if applicable)
• Child’s first and last name
• Child’s birth sex
• Legal guardian’s first and last name
• Home address
• Phone number
