When your child has specialized medical or developmental needs, coordinating appointments, treatments, and follow-up care can feel overwhelming. At Manning Family Children’s, our Complex Care Program helps to bring those pieces together. Our team partners with families, pediatricians, specialists, and hospital care teams to support children through every stage of their care.
Helping families navigate care for children with complex medical needs
Children with multiple medical conditions often receive care from several specialists and may need additional support at home or during hospitalizations. Our Complex Care Coordination team works with eligible children and their families to help make that care more connected.
Services include:
- Proactive outreach: Checking in with families to identify needs and provide ongoing support.
- Specialty care coordination: Helping coordinate appointments and communication among specialists.
- Hospital discharge support: Working with inpatient teams and families to help prepare for a safe transition home.
- Preventive and routine care: Helping families stay on track with recommended health maintenance and follow-up.
- Care team communication: Supporting collaboration among your child's pediatrician, specialists, and other healthcare providers.
Eligibility for Complex Care Coordination is based on medical complexity and review by our team.
A Partner in Your Child's Care
Our goal is to help families feel supported, improve communication among care teams, and make navigating healthcare easier. The Complex Care Program works alongside your child's existing physicians and specialists. Care coordination does not replace your child's primary care provider.

Refer a patient
Parents and caregivers may talk with their child's healthcare provider about whether the Infant Development & Follow-Up Clinic may be appropriate. Referring providers may call to schedule or fax a referral to the numbers below.
