Who qualifies for a weight loss referral?

Adolescent patients with one of the following qualifying BMI conditions:
- BMI ≥ 120% of the 95th percentile for age or BMI ≥ 35kg/m2 whichever is lower, with obesity related health conditions (see list below)
- BMI ≥ 140% of the 95th percentile of BMI for age or BMI ≥ 40 kg/m2, whichever is lower.

Obesity related health conditions include but are not limited to the following:

Type 2 diabetes
Idiopathic intracranial hypertension
Fatty liver disease
Gastroesophageal reflux disease
Obstructive sleep apnea (Apnea-Hypopnea Index > 5)
Joint or mobility symptoms
Cardiovascular disease risks including hypertension, hyperlipidemia, insulin resistance)
Weight related depression

Surgical weight loss
- Vertical Sleeve Gastrectomy (gastric sleeve)
- Roux-en-Y Gastric Bypass (gastric bypass)
- Hiatal hernia repair (when appropriate)
Medical weight loss / obesity medicine
Comprehensive pre- and post-operative care
Nutrition education and dietitian visits
Psychological evaluation and support
Insurance verification and coordination (Note: LA Medicaid will usually approve the surgery. Other insurances are subject to approval).
